Click on “Forms” in the left-hand menu
Find the form in the list and click its title
Hover over the “Settings” button and click “Notifications” in the dropdown
Click the switch to the left of a notification to turn it off
(The white dot to the left means its disabled off; the white dot to the right means its enabled)
